For employers and small businesses: a complete paper/PDF application form covering the sections hiring actually needs.
This is the employer-side form: a complete, printable employment application a small business can hand to walk-in applicants or attach to a job post. It covers the sections with genuine hiring value, availability, work authorization, education, three work-history blocks with duties and reasons for leaving, professional references, and a signed truthfulness/verification authorization, which is the line that makes reference checks permissible and honest answers enforceable.
What the template deliberately leaves out matters as much: no questions about age, race, religion, marital status, health or other protected characteristics, which employers must not ask in most jurisdictions, and no Social Security number field, collecting SSNs on paper applications is an identity-theft risk and unnecessary before a hire decision (it belongs in post-offer payroll paperwork). Adapt wording to local law where your jurisdiction requires specific notices.
I'm a job seeker, is this form for me? This one is the employer's form. If you are applying, the Resume Generator and Cover Letter Generator on this site are the applicant-side tools.
Can the form ask about criminal history? That depends heavily on jurisdiction, many places restrict or ban the question on initial applications ('ban the box' laws). This template omits it for that reason; consult local rules before adding it.
How long should applications be kept? Common practice is at least a year for hiring-record retention, and longer where local law requires. Store completed forms securely, they contain personal data.
